The Health for Life Program (HeLP) CHRONIC CONDITIONS WORKSHOP BY PHONE
This free workshop for older adults:
• "Meets" once a week by phone for 6 weeks
• Is designed to help learn to manage chronic conditions
• 4 to 6 others on the phone call
• Led by a trained facilitator
When: Six Wednesdays from 2 to 3 pm
Oct. 13,20,27, Nov. 3,10,17
The workshop is free but pre-registration is required.
